I tow a Wrangler with rear mounted spare... Thought it would be an issue to use my Yakima '4 bike' rack - so much so that the car dealer threw in his used '2 bike' rack that he knew would work when we bought the Jeep. Turns out the Yakima works just fine too - no extra extensions needed to clear the spare tire.
I've had 3 bikes hanging off the back (honestly difficult to get 4 bikes on a '4 bike' rack') with no issues.


The 'dual take off extension' is certainly an option if rated properly for what you are towing. It will move things back a bit - so if cable lengths are close, might need to replace/extend too.
