Exclamation shore power socket failure

Checked to see why the shore power socket was held by only two screws today....oooopps The socket was installed off center so two screws were holding air.
But that wasn't the worst thing. The ground wire had shorted with the hot wire at some time and burned off. The original installation had to much of the outer insulation removed so the strain relief clamp pinched the hot and ground wires resulting in the short. All fixed now and replaced zinc screws with SS.

I'm not sure how that didn't manifest sooner. It should have arced when plugging into shore power or if a high resistance short, barely touching, the cord should have been hot.
Do you use an EMS ? It seems the fault should have been picked up. But maybe not down stream.
