The second certainly sounds like a warped brake rotor.. Does it happen when you first start? or after driving (braking) warms things up?
If after warming up - I would definitely suspect the rotor...

An event that could result in a warped rotor...
A frozen caliper could cause this... I would have expected a strong pull on the steering wheel if a 'single wheel' event.
An emergency brake issue could have engaged both sides...

If it is indeed warped - it will need to be replaced... Resurfacing is done cool - the newly turned rotor will still warp under heat.

Not sure how the other symptoms tie in..
