All the ones I've received have been via sending Thor an e-mail from their website.
Go to the Customer Care link below and click on "Customer Care" at the left:

I've found that a short non-confrontational note works best. I'll typically get a reply within 3 days or so with an attached .pdf file.
My last request was something like:
