A tall friend with an Axis had his seat frame extended at the top and an upholsterer extend the cover. He's happy with the modification.
We are all built differently, but we have found that those mesh lower back support things work well for us. With that, I can have the seat more vertical, which brings the headrest closer. Here's a pic from the web:
